Sharecare is the SHBP wellness provider. Sharecare provides well-being and incentives for members that complete certain activities throughout the plan year. You and your covered spouse are able to earn incentive points that can be used to receive a Visa gift card or to offset your out-of-pocket costs for medical and pharmacy expenses. Points are saved in your BeWell account until redeemed by the member. Activities include:

1. 1. Well-Being Assessment (Real Age Test): A confidential, online questionnaire about your health. Earn up to 120 pints

2. Biometric Screening: Assesses your health. Earn up to 120 points

3. Take Action with Coaching or Online Pathway: Phone coaching can earn you 60 credits for one call in a calendar month up to 4 times / year. Online Pathways can earn you up to 120 credits within a 90 day period. You can complete two events, for a maximum of 240 credits.

Pharmacy Information

CVS Caremark administers the pharmacy benefits for all medical plans.

· Members do not have to go to a CVS pharmacy location for prescriptions. The CVS Caremark network is extensive and participating pharmacy information is located at: Info.caremark.com/shbp

· If you or a covered dependent are enrolled in the SHBP Disease Management Program, then certain drug costs are waived for diabetes, asthma, and/or coronary artery disease. SHBP must be the primary insurance coverage on file. For more information, click here to visit the Sharecare website

Live Health Online - Telemedicine

As a participant in the State Health Benefit Plan, you are able to access medical professionals virtually through telemedicine 24 hours a day/7 days a week. HMO participants pay a copay and HRA members pay coinsurance for telemedicine visits. High Deductible Plan Members are subject to the deductible.
